Respirator Hazard Assessment Form

The Respiratory Hazard Assessment form is used to request EHS perform an evaluation of work you or your staff perform using chemicals, biological agents, or hazardous dusts to determine if respiratory protection is needed. EHS will perform exposure monitoring as needed to make this determination.

An alternative (preferred) process is to complete the RHA request form found under the Medical Services tab in the Safety Management System. If you have not set up a lab or workgroup in the SMS, go here, to do so. When you are creating your lab/workgroup, be sure to check the 'Medical Services' box.

If you have already created a lab/workgroup in the SMS and do not see the Medical Services tab, click on the "Summary" tab and then the "Edit Lab/Tabs" button. Check the box beside Medical Services and then click on the "Submit" button at the bottom of the page.
