Accident Investigation Training

Supervisors/Departmental Safety Representatives

All supervisors, principal investigators, safety representatives, and/or designated departmental responsible persons should attend Accident Investigation training.  This training will provide the tools and background necessary to determine incident causes and prevent recurrence. 

In any incident investigation, fairness and impartiality are absolutely essential; otherwise, the value can be destroyed if employees suspect its purpose is to place blame or find a scapegoat.  Investigations must be conducted fairly and with a focus of gathering evidence solely for the purpose of preventing a recurrence of such incidents.

Additional training in EHSS programs would also be beneficial in identifying hazards and determining appropriate controls and protective equipment, which may play a factor in the incident being investigated.
