This form is for contractors, A&Es, vendors, or other visitors who must enter university confined spaces in order to review, inspect, or perform other necessary work where there is no agreement with the university at that time to perform such work.

Visitors who need to enter university confined spaces must have received proper training through their employer, have a written confined space program in effect, and coordinate activities with the university prior to entering.
